At 0810 hrs on April 3, 1968 2nd Platoon Alpha Company 4-3 Infantry tripped booby trap (apparently hand grenade) vicinity BS525888 resulting in 5 WHA Dusted Off. At 1815 hrs Charlie Company 26 Engineer with mine detector accompanying 2nd Platoon tripped a Booby Trap mine in the (Bouncing Betty) vicinity of BS525888 resulting in 1 KHA with 10 WHA 3 evac and other remained with Unit. At 1448 hrs complete casualty results SGT Bellomy broken leg a Priority, PFC Skumurski with frag wounds to leg and head Priority, and PFC Clagget frag wounds in Arm Routine, PFC Neally with frag wounds in buttock routine and SP4 LIS frag wounds in legs. Other personnel with mine wounds (not Evac) LT Anderson, SGT Daigneault, SGT Cooper, PFC Kastern, SP4 Meyers and SP4 Cerda. The 3rd Platoon Charlie Company, 26 Engineer personnel evac Dust Wright, Paul, Gartey with minor wounds not evacuated SGT Helper.

E-4 Peters, George Evac wounded in both legs and arms and E4 Akutgawa, Mark has frag wounds in back member of the Recon Platoon. Recon was returning to the Battalion TOC after cutting down tree’s and clearing the road just outside the perimeter.

At 0115 hrs November 27, 1968 Alpha Company at grid BS932222 received mortar rounds, 1 KHA and 1 WHA evac, ceased incoming at 0130 hrs 12 rounds mortar unknown size. Corporal Jimmie Green was serving as a rifleman with the 2nd Platoon; Company A, 4th Battalion, 3d Infantry, 11th Infantry Brigade when the friendly force was subjected to an intense and accurate hand grenade attack, followed by heavy small arms and automatic weapons fire.While grenades were exploding throughout the friendly position, Corporal Green noticed two live grenades land within a few feet of one of the perimeter positions. With complete disregard for his own safety and braving a hail of enemy fire, he ran from his own position and knocked one of his comrades to the ground, covering him with his own body to shield the soldier from the devastating blast of the grenades. Two explosions followed immediately and fatally wounded Corporal Jimmie Green. His unselfishness and his concern for others saved the life of one man & possibly the lives of other. Corporal Jimmie Green's conspicuous gallantry,extraordinary heroism and intrepidity at the cost of his own life are in the highest traditions of the United States Army and reflect great credit upon him and upon the Armed Forces of his country.

At 1500 hrs. on February 11, 1969 LATE ENTRY AlphaCompany 4/3 Infantry at 1425 hrs. requested a Dust Off in the vicinity of grid BS486848 for 7 US WHA and 1 US KHA. When the 2nd platoon hit an unknown size mine, while conducting a sweep operation in LZ Buff AO. Second platoon had advanced to a small hill (w/a30 degree incline) for a break (ground showed signs of old GI garbage on ground). Mine was pressure detonated and was a Bouncing Betty planted on side of hill with old c-ration cans on ground. Dust Off completed at 1455 hrs. Personnel involved; Hill, Michael SP/4-leg wounds... At 2000 hrs on February 26, 1969 LATE ENTRY At 1600 hrs the Command Post for the PF and the US Advisors were pinned down by automatic enemy fire at coordinates and
BS675775 and the Advisors call Alpha Company for help. At 1600 hrs the order was given for Alpha Company 4-3 Infantry 3rd Platoon and 3 APC to move from Chic and reinforce at the contact area. The 3rd Platoon and 3 APC departed the CHIC at 1615 hrs. Alpha Company CO and advisors observed action from Buddha Mountain and co-ordinated Artillery from there. At BS668753 Alpha Company 3rd Platoon and APC received incoming mortar and rocket rounds while dismounting the APC the 3rd Platoon had 2US KHA and 1US WHA, APC had 1 US WHA. At 1650 hrs the 3rd Platoon and APC moved forward and linked up with the US Advisors and found 3 of them WHA and out of Ammo. At 1700 hrs to 1710 hrs contact with the enemy broken. At 1905 hrs on September 21, 1969 Alpha Company 4-3 Infantry in their previous used ..Night Defense Position received 30 82mm mortar rounds at BS577414 and small arms fire from all around perimeter.Alpha Company reports 4 US KHA and 5 WHA. At 2000 hrs Alpha Company update reports add 7 more KHA and 6 more WHA for a total of 11 KHA and 11 US WHA. First Dust Off for 6 US WHA completed at 2000 hrs. At 2100 hrs Alpha Company reports the 10 KHA and 10 WHA are Dusted Off with 1 WHA remaining in the field. There where 3 mortar rounds that landed directly in fox holes resulting in all KHA. Element set up their NDP in an old NDP position....(enemy had area measured) Personnel involved KHA Foster, Joe A. At 0800 hrs on December 24, 1969 Alpha Company 4-3 Infantry vicinity 645449 observed 2 NVA/VC in wood line engaged with small arms fire enemy was 50-75 northwest of ND Position traveling northwest. The 2nd Platoon received small arms fire as they where sweeping the area and engaged with small arms fire from unknown size enemy force. At 0835 hrs Alpha Company vicinity BS645449 requested Dust Off for 2 persons(1 US KHA, 1 US WHA) KHA Dennis, Ronald G. SGT gunshot wound, WHA Collins, Clyde R. PFC-bullet graze wound forehead. Sharks with Dust Off completed at 0910 hrs. At 0915 hrs Sharks continued to provide air support and search of area. At 0940 hrs Alpha Company requested Dust Off for Dempsey, Peter C. PFC involved in earlier engagement and suffered minor eye injury from ricochet rock fragments. At 1000 hrs Alpha Company reports found 1 NVA KIA with AK-47, 1 empty magazine, pack with food, 1 bag of Opium, wore blue uniform with red and yellow arm band with initials “TB” and there was many blood trails.

At 0815 hrs on January 19, 1970 4-3 Infantry TOC on LZ San Juan (grid BS635382) Emergency Dust Off requested by Battalion Medic for 3 persons injured by exploding round believed to have been a M-79 round in burn pit, have 2 men flag wounds, 1 man with severe burns, incident in 81mm mortar section of Alpha Company 4-3 Infantry ...Dust Off was completed by 0835 hrs. Personnel involved Mitchel, Juan SP- burns on face and chest and Turner Van PFC- severe frag wound in chest. At 1015 hrs 4-3 Infantry TOC was informed by Doctor Control that Turner, Van PFC, was Dead on Arrival at the 23rd Medical at Doc Pho.

At 1325 hrs on March 29, 1970 Alpha Company 4-3 Infantry 1st Platoon at BS624497 while moving from their Patrol Base down trail toward the west engaged 1 NVA/VC believed to be a trail guard for VC/NVA base camp engaged with small arms fire, 81mm mortar and flame thrower. Results 1 US KHA Ackerman, Bill R PFC was hit several times in chest from small arms fire. At 1503 hrs Alpha Company at BS622497 received small arms fire from above grid with results 3 US WHA and Dust Off call and arrived is receiving fire and using basket unable to complete. Alpha Company found another LZ and completed Dust Off at 1637 hrs. Personnel involved WHA Mc Cornick PFC- shotupper groin, Kitch PFC -hit in left knee and Fossett, Robert SGT- scrapnel and shock.

Believe booby trap was made of old smoke grenade filled with explosive, was detonated by trip wire. Was place above ground and left no crater placed beside the trench that the element was moving in results One man hit in upper chest, neck, face, one man hit in upper thighs, one man hit in lower legs. One of the above was DOA Personnel WHA Blanchard, Larry F, Bryan, David A, SP4 (Dead on Arrival) and Smith Scott K PFC, Dust Off requested at 1130 hrs and completed at 1200 hrs.

At 1520 hrs Alpha Company 4-3 Infantry element spotted and engaged 1 NVA with small arms fire who evaded to bunker, frag the bunker with results 2 NVA KIA and 2 AK47 captured and misc documents and a medical kit. At 1540 hrs to 1755 hrs on February 14, 1971 Alpha Company 4-3 Infantry 1st Platoon at XD927547 received approximately 30 82mm mortar rounds and we have 2 US WIA, 2 rucksacks destroyed. At 1040 hrs Alpha Company 4-3 on July 19, 1971 Alpha Company 4-3 Infantry 2nd Platoon at BT194125, early this morning on Visual Recon, spotted two poles with what looked like blue shirts was spotted. They were about 100 meters apart. Alpha Company 2nd Platoon was inserted near this area in a Combat Assault and the shirts were blankets very large and directly over bunkers. Element moved in and started checking and swept area.They received fire from 200 meters to northeast. Estimate enemy force to be about 15-20 VC/NVA and resulted in 1 US KIA and 4 WHA. Personnel involved KIA SP4 Maxide, Norman, WHA SP4 Fields, Carlton, SP4 McHugh, Michael, SP4 Millard, Robert, SP4 Cordes,Mack.Late Report in the area there were large rock with fighting position 4’ X 4’ between rocks there are between 10-12 (of these positions in the area).

Point man noticed 3 sticks were in a circular pattern before booby trap detonated. A click was heard prior to blast. Suspect claymore or M-79 round with trip wire and results 1 US KIA and 2 US WIA extracted bylift ship to 91st Evac slight wounds suffered by 2 other US and 1 New Zealand VIP (Major Ogiloy).
