Bravo Company has line # 48 SP4 Harder Rickie L., has a sprained ankle and line # 143 SP4 Silvio, Louis M. was bitten on hand by unknown insect: hand is swelling...Both are to be brought to Shepherd by resupply slick.

Delta  Company CP/at grid 928431.....element had a mech detonate resulting in 1 small animal.

April     1     1840

From Bde/Reference incident of friendly fire, 4 men of the 502 were dusted off. 1 was released with minor wounds. 3 were doing fine. No one was seriously wounded from the 502 unit. The 502 unit was confused on the boundary line,They moved down the ridge line not knowing they were approach -ing another AO.

April     2     0730

Equipment found by C/2-1 at grid XD955451 1 sensor TRC-3A (550335, 4 WP smoke, 20 rounds 7.62 linked, 5 US frags, 500 rounds M-60. 6 claymores, 6 smokes, 1 pound of C-4, 1 hand flare, 10 flares with gun, 50 mig-mag, 1 pocket C grenade, 3 trip flares, 2 M-79 HE rounds, 1 Soviet light machine gun drum, 20 rounds 7.62 and 1 US rucksack.

From LP/C/2-1 at grid XD955451 ...element found 4 chicoms and 1 LAW. There are 3 trails in the area/ 2running NS and 1 running EW.Found 60 loose frags and 1 case of unopened US frags.  Will destroy in place..

Dust off requested for 2/A Co...Man with self-inflicted gunshot wound in  the foot: First initial of last name B....man dusted off to 18th Surgical Hospital in Quang Tri.

April      2     1815

From Bde at 011615 ......20-30 NVA were  spotted in open area at 725477. At least 10 NVA were KIA at 021645. The equipment was evacuated. Element was identified as the 35th Sapper Battalion and the POW that was captured said they were to hit Khe Sanh this evening. All elements should be on alert for possible ground attacks.

April     3     0915

From CP/B Co at grid 942454 element found two bunkers 4' x 4' and 5' x 5' with 2' overhead cover ( logs and dirt ). Element found 15- 82 mm rounds partially burned, 1 chicom canteen with shrapnel holes, 1 US pistol belt with ammo pouch, 3 plastic bags with blood on them, 1 NVA cleaning kit for a  AK-47, small region11can of PETN explosive , 3- 82 mm mortar fuses, and 1 demolition bag (US). On a trail heading south , they found a chicom claymore fixed in a mechanical ambush with a trip wire trigger. Everything will be destroyed.

From Bde/weather report for military  region 1 : thunder storms, winds 15-40 knots. Visibility 1000-3000 broken, 3000' and overcast.

April     3     1605

From 1/2/C Co at grid 950453 ...Light patrol found an estimated company size base camp. Last used one week ago.There were 20 bunkers size 6' x 4'. One bunker was used as a mess hall. Size of the camp 100m X 75m....Equipment found: 22pairs of old tennis shoes, 4 old NVA  ponchos and 21 .51 cal rounds.

April     3      1740

From convoy (jackal 48) had a S+P go off embankment. The driver is pinned under the cab. They need something with heavy lift capability....Suggest hook  or crane. Request that a medivac stand by if the man is still alive. They will need a M-88 to recover the truck. (driver died at scene)

Co C request for line # 36 SP4 Daro Karl M. with a temperature of 103.2 degrees.Passed to pad to relay S-1. Man dusted off and brought to Shepherd by resupply bird.(transferred to Quang Tri).

April 4 1600

To 2-1 Inf/Sensor team entered their AO at grid 918418 and will proceed west for 1 km along draw. Will blow claymores and pick up remainder of equipment. Will depart same route....

April 4 1620

Weather warning 041200-042200 thunderstorms over military region 1...15 knots to 30 knots: min conditions ceiling 1000' broken 10000 overcast.

April 5 0715

Cav unit ambushed at grid 941418.. Estimate NVA platoon or company size, reacted with SAF, 50 cal, 4.2 mortar. Received SAF, 10-15 RPG. Elements of A Co alerted to move to ambush site. 2 US WIA, 1 track hit by SAF. Area swept twice by Cav unit.

April 5 1105

From C/2-1 at grid 958434 spotted 1 NVA lying on the ground who appeared to be dead or seriously wounded.There were 4-5 NVA standing around him. Attempted to fire arty but was check fired due to CA of Pink Team and A4/3. Results 1 NVA KRA due to scheduled fires this morning.

April 5 1215

Late Entry: From 4/A Co/ at grid 941435 ...Element spotted 1 NVA and engaged with SAF. Swept area finding 1 heavy blood trail heading south. While following blood trail, element was ambushed by 2-3 NVA with SAF and chicoms, NVA evaded to south, 1st platoon is moving to intercept them. Also NVA had hung cardboard on trees along the trails leading toward the road. Captured 1 RPG launcher, 3 US WIA (minor) 1/C Co is following blood trail now.

April 5 1130

Names of WIA from 4/C Co passed to pad to relay to S-1: line # 41 Gilson, Daren L. ; shrapnel wounds to the neck: line # 67 Arcori, John A. ; shrapnel wounds in leg: Schwab G. ; shrapnel in left cheek.

April 6 1850

Man dusted off from 1/A Co due to temperture of 104 degrees........Shepherd to Dong Ha.

From 3/A Co DTG 161120 at grid 457502....Element was retrieving mech ambushes when they were engaged by 2-3 VC/NVA with a crew served weapon at a distance of 150 meters. The enemy evaded to the south. Results: 3 US WIA, enemy casualties unknown... Dust off requested...Arty employed. Element will sweep contact area. Initial spot report at 1125 hrs.

April 16 1230

Dust off for 3 US WIA...A Co line # 121 PFC Hernandez, Delgado J, 2 small arms wounds in leg: # 47 SP4 Bednar, Martin J, SAF in hand: # 86 SP4 Schultz, Sammy L, SAF in hip. Dust off complete 1155 ...Passed to Bn rear at 1230.

April 16 1535

From 1/3/C Co DTG 1611535 at grid 456455....element had a mech detonate. Results 1 VC/NVA KIA. Elementing still investigating.

April 16 1905

Request dust off for mortar platoon McMorrow, James ....swallowed plotting pin. Dust off completed 1930.

From 1/3/B Co DTG 162005 at grid 417497....element received 10-30 rounds of AK-47 fire. Results 1 US WIA. Seemed to be probing fire, WIA was hit in arm and leg. Dust off not required until morning. SAF came from 100 meters to the south...Arty fired...

April 17 0655

Dust off requested for line # 10 of B Co SFC Van Valkenburg, Hobart, completed at 0720. Refer to spot 172005. 1/3 B Co at grid 417498. Line # 135 PFC Savino, John M , a self inflicted wound in the foot while waiting for another dust off. Statement for wound requested by CPT Mendonca.

PFC Mooney, James was extracted to 23rd Med by resupply slick due to blood poisoning.

April 18 1900

From 1/3/D Co DTG 181615 at grid 438501...Element made contact with 2 VC/NVA who were moving N on a trail. Element engaged them at 30 meters. Results; 1 VC/NVA KIA, 1 VC evaded to the SE. He was by 2/3/D Co and killed. Result 2 VC/NVA KIA. CIA; 1 AK-47 w/14 rounds of ammo, 2 rucksacks, 1 first aid kit, 1 NVA pistol belt, 1 NVA officer's belt, 1 pair of black pj's, 2 NVA ponchos, 2 sets of extra clothing, green and documents. The VC/NVA were wearing bright green long sleeve shirts and pants. One had a NVA helmet. Equipment and documents will be back hauled to LZ Tiger.

Forecast DTG 191800-201800; fair to partly cloudy with patchy fog in the morning. Ceiling 2500 broken, 7000' overcast, visibility 7 miles. Winds NE at 12 knots. High 83 degrees, low 74 degrees, humidity high of 90% to low of 65%. Forecast little change.

April 20 0740

B Co CP received 30 rounds of AK fire from 60 meters to south of position. Returned fire with M-79 and M-16 fire. Believed secondary explosions with M-79 fire. Received 10 rounds AK fire 20 meter further south. Appeared to be 1 enemy involved. 50 rounds Arty fired...Negative friendly casualties. Will search area... Enemy initiated from grid 432508.

April 20 1218

From 1/D Co DTG 221150 at grid 421464. Element found 1 hooch size 15' x 10' made of thatched straw containing a wicker basket. Also found a bunker 4' x 5' and a fighting position in the area. Element will destroy hooch and bunker.

April 20 1600

From D Co CPT/CPT Hummel DTG 201600 at grid 426473. LTC Coast was moving toward LOH at above location when he was hit on the top of head by main rotor blade. He was evacuated to Duc Pho by LOH. LTC Coast Albert F. died at 91st Evac Chu Lai of injuries sustained at 1600.

April 20 1640

Request dust off for Dimone Theodore PFC with a temperture 106 degrees. Dust off complete 1700.

April 21 0840

From 3/B Co DTG 210835 at grid 417492. While in NL, element had EM walk into friendly mech.Individual was new in the element. He had been warned the mech was there; Results 1 US KIA ....request slick to extract the body..

Line # 117 on B Co rooster was KIA by mech at grid 417492. La Bounty Charles R. PFC ....B Co body was extracted.

From C Co CP/221015 at grid 457461.At the above location element heard movement. Moved to check it out finding 1 NVA pith helmet, 1 bush hat and 1 sack containing small amount of rice. 1 rice bowl , 1 NVA shirt, 1 NVA pants and several papers.Trails in the area 1E-W 1SW-NE and both are well used. Suspects there were 2-3 NVA in the area moving NE and that they evaded E.

From Bde Intell. with A-2 rating; Quang Ngai offensive activities to start night of 23rd of April and dawn 24th April and continue through 29 April, 21 Rgt has cache sites at ANBU grid 6858 & 6651. 21st NVA Rgt is to attack OP on Ong Do mountain at grid 769531 anytime between 23 April and May 2.

From 4/A Co had individual B(initial) injured leg when a tree fell on him....Extracted by resupply bird for x-ray.

April 24 1320

From 3/A Co line # 31 eye is bleeding internally and will be extracted to LZ Bronco by r/s slick.

April 24 1545

From 4/D Co 241520 at grid 424483 element found 5 bunkers constructed of bricks 6' x 8' x 20' while on patrol, Signs of use in last 7-10 days. Age of bunkers 2-3 year old. Coming in to area spotted rocks marked with arrows pointing toward bunker area. One trail in the area running N-S, Found several papers in area and will be back hauled to LZ Tiger. Bunkers will be destroyed.

From Bn rear/Destefano, ; SSG Tillotson while working on rifle range received shrapnel in arm from demolition of dud round. Is presently in Chu Lai and will be going back to the states.

Weather report from Bde/Nettles; Fair to partly cloudy skies . Ceiling in the morning 2500' scattered. Ceiling unlimited after morning fog lifted. Visibility ; 7 miles Winds ; 5-20 knots E-SE. Isolated thunder showers in the afternoon...Temp; H-89 L-76 Humidity: 90%/60% outlook little change.
